UMID Requirements
Bring ONE valid primary ID:

Passport, Driver’s License, SSS ID, GSIS ID, Postal ID, PRC, etc.
Or bring TWO secondary IDs:

Birth certificate, NBI clearance, barangay ID, school ID, TIN card, etc.
How to Apply

Book an appointment via the My.SSS Portal

Bring your valid ID/s and completed UMID form.

Go to your scheduled branch, have your photo/fingerprint taken.

Wait for delivery (usually 30–60 days).

Lost Your UMID?
If your UMID is lost or damaged:

- Bring a black/blue pen
- Wear decent clothes for the photo (no shades, no hat)
- Keep your acknowledgment stub
- Avoid erasures on the form
- Be early, some branches have long lines
FAQs
Can I apply if I’m not an SSS member?
No. You need to be a member of SSS, GSIS, PhilHealth, or Pag-IBIG.
Is UMID accepted for passport application?
Yes. It's a valid government ID.
What if I didn’t receive my card?
Follow up at the branch. Unclaimed cards are destroyed after 60 days.
